1. Download and print the intake form. Download here...
2. Complete the intake form to the best of your ability
3. Bring the form to your appointment, and provide to our intake specialist

Sometimes, there are more forms that will be required depending on the counselor you will be seeing. Once our intake specialist has matched you with a counselor, you may locate that counselor under the “Counselor” tab (at the top of the website), and any additional forms required will be posted under his or her name. If additional forms are needed, simply follow steps 1-3 above.
